Application Procedures

Application Forms:

Ontario high school students must contact their guidance office to apply to York University. Please use the 101 Application Form provided by the Ontario Universities' Application Centre (OUAC). High school students who are 21 years of age or older are encouraged to apply using the 105D application.

University students transferring from an Ontario university are encouraged to use the 105D Application Form available at all Ontario university admission offices. This form allows you the opportunity to have your transcript sent to us electronically.

All other applicants may apply using one of the following application forms:

1. The 105D Application Form provided by OUAC. This form enables you to apply to more than one Ontario university. It is available on the OUAC Web site www.ouac.on.ca.

2. The York University Undergraduate Application is available through the Web site www.yorku.ca/admissions as an online application or to download and print or through Admissions Enquiries.

3. The 105F Application Form for international students who apply from outside of Canada and are interested in choosing more than one Ontario university. It is available on the OUAC Web site www.ouac.on.ca.

Supplementary Admission Information Form for Schulich School of Business

Submission of this form is mandatory. Please refer to the Web site www.yorku.ca/admissions for the due date and form.

The form will also be automatically mailed to applicants who apply before the deadline.

Evaluations/Auditions for Faculty of Fine Arts

Applicants must satisfy a departmental evaluation and/or audition as a condition of admission to the Faculty of Fine Arts. You are advised to contact the department of your choice to arrange for evaluation/audition. Information on when you should arrange for your evaluation/audition is available on York's Web site www.yorku.ca/admissions. Supplementary Admission Information Form for Bachelor in Environmental Studies

This form is available from the Office of Admissions, and on York's Web site at www.yorku.ca/admissions. It will be mailed to all applicants after they apply. Please refer to the Web site for the due date. International students are encouraged to submit this form as soon as possible due to potential international mail delays.

Supplementary Admissions Information Form for Bachelor of Social Work

This form is provided with the application package available from the Office of Admissions, and on York's Web site at www.yorku.ca/admissions for the forms and due date.

Supplementary Admissions Information Form for Bachelor of Science in Nursing

Submission of this form in mandatory. This form is provided with the application package. The application package and due date information can be obtained on our Web site at www.yorku.ca/admissions, or from the Office of Admissions.
